I acknowledge that there is an active global pandemic of a novel coronavirus known as “COVID-19”, and that exposure to and infection with COVID-19 can lead to serious illness, hospitalization, and death.

I understand that the Creative Arts Center of Dallas (CAC) is open to the public and, in spite of routine and additional cleaning and disinfection, the Creative Arts Center of Dallas cannot guarantee an infection-free environment.

I agree to wear a mask whenever I am on the premises of the Creative Arts Center of Dallas. While this may not ultimately protect me or my family and guests from a COVID-19 infection, I commit to wearing a mask out of respect for my fellow students and teachers, so that if I become an unknown carrier of an illness, I am less likely to pass it to somebody else.

I further commit to immediately notifying Creative Arts Center of Dallas should I or somebody within my close circle of family or friends should become infected with COVID-19.

I assume liability for the risks and dangers participating in classes or workshops may pose to myself, my family, and my guests.

I understand the Creative Arts Center of Dallas may have to cancel or reschedule classes or workshops for any reason including, but not limited to, the protection of students, staff, the public, and/or anybody associated with the Creative Arts Center of Dallas. If this occurs, I may be offered the opportunity to participate in a rescheduled class or workshop on a regularly available future date without penalty.

I understand and agree that the Creative Arts Center shall be the final authority in deciding whether a class or workshop will be rescheduled.

I indemnify, defend, and hold harmless the Creative Arts Center of Dallas and its agents, employees, Board of Directors, and volunteers from any and all claims or liabilities that may arise from infections or an illness that may occur by and through my engaging in classes and workshops at the Creative Arts Center of Dallas facility.
